Coming to Our Office

Thank you for scheduling your appointment with us. We’re passionate about the excellent healthcare we offer women and we’ll do everything we can to make your appointment timely and as pleasant as possible. If it becomes necessary to cancel your appointment, please call and notify us at least 24 hours beforehand. (Click here to read our Missed Appointment Policy.)

Please remember to:

• bring your completed forms.
• bring your insurance card.
• bring a list your medications (including herbs and vitamins).
• arrive 10 minutes before your appointment.

Payment:

Payment is due at the time of your service. For your convenience we accept cash, check, MC/VISA.
